Health Net Coverage and Plans

Individual and Family PPO

With the Health Net PPO product, your family can choose between Preferred Provider Organization (PPO) and Out-of-Network benefit levels on an incident by incident basis. See any provider in the Health Net network without a referral, or go out-of-network with an increase in out-of-pocket expenses.

Individual and Family HMO

The Health Net HMO option offers members coverage at a lower cost than the PPO option. Members choose a Primary Care Physician (PCP) who is responsible for all referrals and claim processesing, from Health Net's network. There are no variable expenses, and out-of-pocket costs remain low.

Group Employer Plans PPO

If freedom of choice is what you want then a PPO is the plan for you since no referrals are required and there is no claim filing for in-network services. You can choose one of the 48,000 doctors in the PPO network or one of the 300 hospitals, or you can see a provider not in our network..

Group Employer Plans HMO

Health Net of California has translated decades of HMO experience into plans that give you a wide range of covered services and predictable costs on one of the largest provider networks in California. The HMO plan gives each member the flexibility to choose their own PCP without filing claims.
